Esophageal hernia, also known as hiatal hernia or hiatus hernia, is a medical condition characterized by the abnormal protrusion of part of the stomach through the diaphragm and into the chest cavity. The diaphragm is a muscular sheet that separates the chest cavity from the abdominal cavity. Normally, the esophagus (the muscular tube that carries food from the throat to the stomach) passes through an opening in the diaphragm called the hiatus, and then connects to the stomach in the abdominal cavity.
However, in esophageal hernia, a portion of the stomach slides or protrudes up through the hiatus and into the chest cavity. This can occur due to a weakened or enlarged opening in the diaphragm, allowing the upper part of the stomach to move upwards.
Esophageal hernias are typically categorized into two types: sliding hiatal hernia and paraesophageal hernia. Sliding hernias are more common and occur when the junction of the esophagus and stomach, known as the gastroesophageal junction (GE junction), and a portion of the stomach slide up and down into the chest. Paraesophageal hernias, on the other hand, involve the stomach bulging up through the hiatus next to the esophagus, while the GE junction remains in its normal position.
Common symptoms of esophageal hernias may include heartburn, regurgitation, chest pain, difficulty swallowing, belching, and anemia if bleeding occurs. Treatment options for esophageal hernias may include lifestyle changes, such as avoiding large meals and lying down after eating, medications to reduce stomach acid or relieve symptoms, and in some cases, surgical intervention to correct the herniation and prevent complications.
